Surgery must be scheduled by December 31, 2024 and performed by June 30, 2025. Offer cannot be combined with any other promotions. In the event that you are ineligible for surgery, the $1,500 surgical credit will not apply, and your $500 gift card may be used for any other services or products that we offer. Gift card and credits will expire by June 30, 2025.
